Author Julia Views Posted at 2014/01/14

Name: Cornelia
Sex: Female
Country: Greece
Age: 30
Diagnoses: 1.Spinal cord injury (SCI) 2. Urinary tract infection 3. fatty liver (mild)
Admission Date: October 23, 2012
Days Admitted to Hospital: 27 days

Before treatment:
Cornelia suffered an injury from a traffic accident 11 months ago. This resulted in movement disorder and sensory disorder of both lower limbs. She, also, had gatism and was sent to the local hospital. The patient was agitated and received an trachea cannula for 4 hours. The Neurologist found the patient suffered from paraplegia below T7. The examination of X ratios, CT and MRI showed multiple cervical vertebra stable fracture. T8 thoracic vertebra had compression fracture. So the patient received titanium plate USS-fixation (from T6 to T10). After treatment, the patient received rehabilitation and electric stimulation therapy. The movement and sensory of both lower limbs had some recovery. 1 month ago, the titanium plate was taken out. At present, the muscle strength of lower limbs is level 1, and has part sensory.

Nervous System Examination:
Cornelia was alert and she was in good spirits. The speech was clear and distinct. The memory, calculation abilities and orientation were all normal. Both pupils were equal in size and the diameter was 3.0mms. Both eyeballs could move freely and the pupils reacted normally to light stimulus. The forehead wrinkle pattern was symmetrical. The tongue was centered in the oral cavity and the teeth were shown without deflection. The neck had flexible movement. She had strong muscles to turn neck or shrug shoulders. The muscle strength of the left upper limb's flexor was level 4, other upper limbs' muscle strength was level 5. The muscle strength of the trunk below T7 was weak. She could sit with support. The muscle strength of both lower limbs was level 1. There was obvious muscle spasm. The muscle tone of both upper limbs was normal. The muscle tone of both lower limbs was higher than normal. The tendon reflex of both upper limbs was normal. There was hyper-reflexia of the tendon reflex of both lower limbs. The abdominal reflexes were disappeared. The pathologic reflex of both upper limbs was negative. The Babinski sign of both lower limbs was positive. The sensation above left T8 was normal. There was current sensation below left T8 segment. The deep and shallow sensation above right T10 segment was normal. And the deep and shallow sensation below T10 segment was slowed down about 50%. She was able to do the finger-nose-test and the rapid rotation test normally. She was unable to finish the heel-knee-shin test. The laboratory test showed: white blood cells were full of visual field. The ultrasound of the abdomen showed she had fatty liver.

Treatment:
We initially gave Cornelia a complete examination and she was diagnosed with a spinal cord injury and urinary tract infection. Then we proceeded with the nerve nutrition, neural repair, nerve regeneration and improved the blood circulation in order to increase the blood supply to the damaged neurons. This was combined with daily physical rehabilitation training.

Post-treatment:
The muscle strength four limbs has improved. The muscle strength of both upper limbs is level 5, the muscle strength of both lower limbs is level 2. The spasm of the muscles of both lower limbs is obviously alleviated. Both knees can lift off the surface of the bed 10cm. Both lower limbs do adduction and abduction in knee flexion. And she can stretch and straighten legs from bending position. The muscle tone of both lower limbs is obviously reduced. The patient suffered from a urinary infection but the infection is alleviated after the anti-inflammatory treatment. Recheck of urine routine showed white blood cells has recduced, occasionally been seen. Abnormal sensation of current through has disappeared.
